Confirmation & Enrollment

Enrollment Requirements
The following documents are required for new student enrollment. Detailed checklists are provided upon inquiry.
Required Documents
- Original Birth Certificate (PSA) of the student
- Form 137 / Transcript of Records (for transferees)
- Report Card / Latest Grades (for transferees)
- 2×2 ID photos (3 copies)
- Certificate of Good Moral Character (for transferees)
- Parent / Guardian Valid ID
